The hawk-i program has health plans, or Managed Care Organizations (MCOs), that you get to choose. Learn more about the health plans by viewing the MCO Benefit Summary Sheet for hawk-i.

*The Department has put in place a temporary suspension of choice. New members will be assigned to UnitedHealthcare and will be notified when choice is available.

Members who are currently enrolled with Amerigroup or UnitedHealthcare will be able to change their MCO during their next annual choice period or for reasons of 'Good Cause.' To verify your annual choice period, please call hawk-i Customer Service.
