Aims and Goals of the Iowa SIM Program


The vision of the Iowa SIM Program, "Transforming Health Care to Improve the Health of Iowans" is achieved by a direct correlation to three primary aims:
 

1. IMPROVE POPULATION HEALTH
Iowa aims to improve the health of its population by the end of the SIM test grant (December 31, 2018) in three of the SIM focus areas:

Goals by 2018 to achieve the aim of improving population health - GOAL # 1

Tobacco Use: Increase quit attempt rate by 5.1% 

Obesity: Decrease prevalence rate by 2.9%

Diabetes: Increase A1c test rate for diabetics by 4.1%


2. TRANSFORM HEALTH CARE
Iowa aims to reduce the rates of preventable events (i.e. Emergency Department (ED) visits and inpatient admissions) for individuals in a value-based purchasing arrangement by the end of the SIM test grant (December 31, 2018).

Goals by 2018 to achieve the aim of transforming health care - GOALS # 2 and # 3

Preventable Readmissions: Reduce the rate of preventable readmissions by 20% in the Medicaid and Wellmark population

Preventable Emergency Department (ED) visits: Reduce the rate of preventable ED visits  by 20% in the Medicaid and Wellmark population


3. PROMOTE SUSTAINABILITY
Iowa aims to increase the number of providers participating in Value-Based Purchasing (VBP) and establish a decreased trend in Total Cost of Care (TCOC) by the end of the SIM test grant (December 31, 2018).

Goals by 2018 to achieve the aim of promoting sustainability - GOAL # 4
Value-Based Purchasing (VBP):  Increase participation in VBP in Iowa, by evidence of 50% of Medicaid, Wellmark, and Medicare payments linked to VBP contracts
