What are Healthy Behaviors?

Members in the Iowa Health and Wellness Plan receive free health coverage in their first year of eligibility. Members must complete their Healthy Behaviors in their first year, and every year after, to continue to receive free health services the following year. There are two Healthy Behaviors requirements you need to complete each year.
  1. You must complete a Health Risk Assessment
​This is a survey that asks questions about your health and your experience getting health services.

You can complete your Health Risk Assessment:
  1. By calling Iowa Medicaid Member Services at 1-800-338-8366 or 515-256-4606 (when calling from within the Des Moines area), Monday through Friday, 8 a.m. to 5 p.m. For telephone accessibility assistance if you are deaf, hard-of-hearing, deaf-blind, or have difficulty speaking call Relay Iowa TTY at 1-800-735-2942.
  2. Online at: www.assessmyhealth.com
  • You will access this tool by entering your provider's five digit access code.
  • You will need to get the five digit access code from your provider to complete the survey online. The access code allows your survey results to be shared with your provider.
  • If your provider does not have a code, enter: MBR11
  • The initial login screen will confirm your physician or physician group before allowing you to complete the HRA.
  1. You must get a Wellness Exam -OR- a Dental Exam
Wellness Exam
In a wellness exam, your healthcare provider will do things like check your blood pressure and pulse, listen to your lungs with a stethoscope, recommend preventive screenings or take a blood sample to check your cholesterol. You may receive an annual preventive wellness exam from any enrolled Iowa Medicaid provider:
  • Physician
  • Rural Health Clinic (RHC)
  • Federally Qualified Health Center (FQHC)
  • Advanced Registered Nurse Practitioner (ARNP)

Dental Exam

In a dental exam, your dentist will go over your dental health. You may receive a cleaning or basic x-rays.

Why do I have to get a wellness exam or dental exam when I'm not sick?

An annual wellness exam or dental exam are aimed at preventing health or dental problems before they happen.
 

I am in the Dental Wellness Plan. Does a dental exam count toward both my Iowa Health and Wellness Plan and Dental Wellness Plan Healthy Behaviors?

Yes.
 

What happens if I don't complete my Health Behaviors?

If you do not complete your Healthy Behaviors each year, you may be charged a small monthly contribution that depends on your family income.
 
Members who do not complete their Healthy Behaviors and do not pay their monthly bill after 90 days, depending on their income, may be disenrolled from the Iowa Health and Wellness Plan.
 

How long do I have to complete my Healthy Behaviors? Do I have to do them by a certain time?

You are given 12 months to complete your Healthy Behaviors. You will need to complete Healthy Behaviors each year to waive your monthly contribution for the next year. 
 
You are also given an additional 30 day grace period when your contributions initially begin to complete your Healthy Behaviors that have not been completed.
 
Example: You were initially enrolled in the Iowa Health and Wellness Plan July 1, 2017. Your initial 12 months of coverage is between July 1, 2017 and June 30, 2018. If you do not complete your Healthy Behaviors, your contributions would begin July 1, 2018. Your 30 day grace period would begin June 30, 2018 and end on July 31, 2018. 
 

I have monthly contributions. If i complete my Healthy Behaviors now, will it count for last year's requirement? Will this stop my monthly contributions?

No. You are able to complete your Healthy Behaviors now but, they will count for the following year. Your contributions would not end until your next annual enrollment date.
 
Members have 12 months to complete their Healthy Behaviors. Members are given an additional 20 day grace period when their contributions initially begin to complete their Healthy Behaviors.
 

I have completed my Healthy Behaviors, why did I receive a statement?

It is possible that Iowa Medicaid does not have information on the completed activities. To discuss your Healthy Behaviors, please contact Iowa medicaid Member Services at 1-800-338-8366 or 515-256-4606 (when calling from within the Des Moines area), Monday through Friday, 8 a.m. to 5 p.m. For telephone accessibility assistance if you are deaf, hard-of-hearing, deaf-blind, or have difficulty speaking call Relay Iowa TTY at 1-800-735-2942.
